To update your existing install:

Make backup copies of your tms files and your database. You won’t need to backup the media files.
Upload the admin folder.
Upload new config.php and change your database settings.
Upload encoder.php, scheduler.php, taskmain.php, tms_core_clear.class.php, tms_core.class.php,
tms_admin_core.class.php.
Upload mainbox_medialist.tpl, sidebox_media_toprated.tpl, sidebox_media_newest.tpl
Upload all the png files in the /media/thumbs/
Upload all the files in the /php/
Upload /templates/pixeled/config/style_Blue.css
Run the sql file in myphpadmin.
Run http://yoursite.com/convert_date_bigint.php.
Run the sql command in create_t_media_types.zip.
Go to admin> manage>languages and click on Import Missing Keys and Generate Language Files.
You should be done.
